PAYMENT OPTIONS
• Player Fees are non-refundable
• Uniform costs are not included
• You will choose a payment plan at the time of registration. When you sign up for a payment plan, you must have a credit card on file.
• There is a $15 late fee for all payments received after the scheduled due date.
• There is a $30 fee for all returned checks or denied automatic transactions
• Payments not received on time will result in the pulling of the player’s pass, making the player ineligible to participate in team activities, including training and league/ tournament games
• Credit card payments will not be accepted over the phone
• The Club’s Operations and Management structure fees to ensure a quality product.
Release Fee: A release fee of $500.00 is required for all players to be released from Miami Athletic Club Academy before the end of the seasonal year. In addition to the release fee, all player fees must be paid in full before releasing the FYSA Player Card. After December 1st, all releases will be issued at the discretion of the Club Director.
Scholarship/Financial Aid: Financial aid applications are due at the time of player registration. All scholarships and Financial Aid must be requested no more than 12 days following your roster spot. A scholarship application will be available upon request. Please consult with the Club Director before registration. All applications must be completely filled out, and income verification must be submitted with the application. Scholarship approval may take up to 2 weeks. No player will be registered with Miami Athletic Club Academy until their scholarship application has been approved and the scholarship contract is signed by the parent/guardian, along with the submission of the initial payment. All scholarship/financial aid recipients will be required to complete a predetermined number of club service hours.

UNIFORM PACKAGE
